We will bill all insurance carriers; however, if we are not in-network with your carrier, you may have a higher copay or out of pocket expense. If you are uncertain whether we are in your plan network, you can contact your plan carrier directly, or call our office to confirm if we are contracted with your carrier. If a referral is required from your primary physician, we ask that you obtain this prior to your appointment.
Copays, deductibles and non-covered services are due at the time of service.
